Thank you for being a patient of Gastro Health. Prior to your appointment, please fill out the new patient and bring them with you to your appointment. Arrive 30 minutes prior to your scheduled time to ensure all paperwork is complete before your appointment. Additionally, please bring your photo ID, insurance card(s), a referral issued by your primary care physician (if required by your insurance), co-pay, and any test or medical results that pertain to your visit. Directions from Route 50 East:

  • Take Exit 13 for US 301/MD 3 North towards Crofton.
  • Drive approximately 7 miles and then make a left turn onto Chapel Lakes Rd.
  • You will find AAMC Health Services immediately on your right.

 

Directions from Route 50 West:

  • Take Exit 13 for US 301/MD 3 North towards Crofton.
  • Drive approximately 7 miles and then make a left turn onto Chapel Lakes Rd.
  • AAMC Health Services will be immediately on your right.

 

Directions from the Eastern Shore/Route 50 West:

  • Take Exit 39A for Castle Marina Rd.
  • Enter the traffic circle.
  • Bear right at the circle onto Main St.
  • Make the first right turn into the AAMC facility.
