Program Eligibility

• Non-employee Gastroenterology Fellows are eligible to participate, including those who have signed Offer Letters or Employment Agreements with Gastro Health.
• There is no limit to the number of referrals an eligible referrer can make.
• The referral must represent the physician’s first contact with Gastro Health and will be time-stamped upon receipt by the Clinical Recruitment team.
• The referring Fellow must consent to having their name used for the introduction.
• The referred candidate must have agreed to the referral.
• Only the first Fellow to refer a candidate who completes an in-person onsite interview will be eligible for the referral award payment. 
• A submission form must be completed in full using the link provided prior to the referred candidate’s interview date. (Link below.)
• The referring Fellow will not be provided details regarding the interview or status of their referred candidate.

Program Terms and Conditions

• Any disputes or interpretations regarding the program will be reviewed and resolved by the Chief People Officer.
• The Company reserves the right to amend, suspend, or terminate the Physician Referral Program at any time.
• Upon the referred candidate’s successful completion of an in-person onsite interview, the referring Fellow will receive the referral bonus within 45 days.
• This policy will be reviewed annually to ensure alignment with Gastro Health’s hiring objectives and needs.
• Referring Fellow will be responsible for any applicable taxes associated with payments received under this program.
• This program is designed to fully comply with all applicable federal, state and local laws, regulations and ordinances.
• To receive a referral payment through this program, you must complete a W-9 form and any other additional required documentation as directed.
